Client information will be gathered during the first visit. You may download the following documents and bring the completed forms to your initial appointment.

HIPPA forms and right to privacy will be explained and signed during the initial session.

I am a provider for many insurance plans and I will submit claims to your insurance company. Please bring your insurance card to your initial appointment. Copayments and coinsurance are due and payable at the time of service. I accept cash, check and most credit cards including HSA and flexible spending cards.
